**In-person Volunteer Questionnaire**

We occasionally ask for volunteers for food packaging in large events: in the daily runs, restaurants do their own packaging themselves, according to the city’s COVID guidelines. In these large events, this is the questionnaire we use with our volunteers:

- Have you been in close personal contact within the last 14 days with another person who has been diagnosed with or who is suspected of having COVID-19?

Yes  
 No

- Do you have any of the following symptoms?

Fever  
 Cough  
 Shortness of breath or difficulty breathing  
 Chills  
 Repeated shaking with chills  
 Muscle pain  
 Headache  
 Sore throat  
 New loss of taste or smell  
 None of the above

- Have you been confirmed COVID-19 positive?

Yes  
 No
